Included here are forms and payment options for you to access and review before your office visit. We are delighted you are considering Northern Sun Family Health Care for your healthcare and midwifery needs. An important element of midwifery and naturopathic medical care is the establishment of a warm and open relationship. And, as natural healthcare is based on treating the whole person, it is important that we have as much information as possible to provide personalized medical care. Please complete the relevant forms below before your visit, and bring these forms along with any questions you have to discuss at this visit.
For expecting families, we feel it is very important for both partners to be present at the initial interview, so please make every effort to schedule an appointment you both can attend.

PAYMENT AND INSURANCE COVERAGE
A number of insurance companies cover naturopathic services, birth center and laboratory fees. Please contact your insurance provider to determine if they cover naturopathic medicine and midwifery services. In Maine, naturopathic doctors are considered specialists. We are contracted as “in-network specialists” with ANTHEM, CIGNA and MAINE COMMUNITY HEALTH OPTIONS for naturopathic services and we only provide billing for these carriers for your naturopathic visits. For all other insurance providers we are considered out-of-network specialists. We are happy to provide an invoice from our office for you to submit to your insurance provider for reimbursement.
For midwifery services, we are not contracted with insurance, and we request payment at the time of service and would be happy to provide an invoice from our office to submit to your insurance provider for reimbursement after delivery.
At this time, naturopathic doctor services are not covered by Medicaid or Medicare. However there are some labs that can be run in our office for those who have these insurances.

A business in the Mid-coast region that is offering this growing service is The Birth Center at Northern Sun, located in Topsham, and its practitioner Sarah Ackerly, a certified professional midwife and naturopathic doctor.
“The Birth Center provides a home-like and beautiful setting for families to come and birth,” said Ackerly, the owner of the business. “It is a safe and roomy environment where clients have the space, physically and emotionally, to allow for their birth experience to unfold.”
